Host Home Providers – Adult Residential (DD)
Our host home providers are independent contractors. They consist of individuals or families in the community carefully selected by Eleos Services to provide a safe, supportive, and caring home for an adult with an intellectual or developmental disability receiving Medicaid Developmental Disabilities (DD) Waiver residential services. It’s a beautiful way to open your home and expand your heart.
Residential Services
Eleos Services is approved to provide the following services under the DD Waiver: Day Habilitation-Supported Community Connections, Transportation and Residential Habilitation-Individual Residential Services and Support. Our focus here is a safe, supportive, and caring home for an adult with an intellectual or developmental disability.
Care in the Home of a Service Provider
We are focused on long-term caring relationships. Match making is not our primary business, we don’t have a long list of “open beds” just waiting to be filled. We will occasionally work with an exceptional host home provider we are familiar with. We will consider partnering with a family and host home provider that already have an established relationship.
Around-the-Clock Care
Eleos Services is about providing 24/7 care. There is a member of the management team available to our care givers around the clock. Nursing support is available to caregivers around the clock.
