Family Caregiver – Adult Residential (DD)
An adult with an intellectual or developmental disability can receive Medicaid Developmental Disabilities (DD) Waiver residential services cared for by a family member in their own home. Family Caregivers that become an Independent Contractor with Eleos Services ensure the highest quality of care for their family members and the best support for themselves as the paid caregiver.
Residential Services
Eleos Services is approved to provide the following services under the DD Waiver: Day Habilitation-Supported Community Connections, Transportation and Residential Habilitation-Individual Residential Services and Support. Our focus here is a safe, supportive, and caring home for an adult with an intellectual or developmental disability.
Care in the Family Home
We want to see your family be successful. Grandparents, parents, siblings can all be delightful primary caregivers. We can work uniquely with your family if you want to share the responsibility of care giving between beloved family members.
Around-the-Clock Care
Eleos Services is about providing 24/7 care. There is a member of the management team available to our care givers around the clock. Nursing support is available to caregivers around the clock.
